
Patient Supplemental Support
The Foundation supports hospice-eligible patients (as determined by provider evaluation), of limited financial means, meet their basic needs that aren’t covered through insurance. Social workers make the requests to the Foundation on behalf of patients.
Basic needs may include: housing, utilities, transportation, food, clothing, safety items (bed alarms, etc.), caregiving, and final arrangements (cremation or the equivalent amount towards burial assistance).
Patients, families and requesting organizations are encouraged to contribute towards the request by funding what they are able to, as the Foundation receives more requests than we are able to cover each year.

End of life decisions are often made in the middle of crisis, when emotions are running high. Imagine the stress that could be avoided by having talked with our loved ones about what we want when “our time” comes. No matter what age you are, accidents or illness can strike when least expected. To help make the process less overwhelming, Casa de la Luz Foundation provides a document called Five Wishes, free of charge, to help you and your loved ones think through all of the choices that will need to be made. This advance directive document is legally recognized in multiple states, including Arizona, and clarifies medical, spiritual, emotional, and personal needs.
Casa de la Luz Foundation was established in 2000 as a nonprofit organization to support end of life care.
